JOB SUMMARY
Assists patients that need a variety of social service interventions. Conducts psycho-social assessments and makes recommendations for post hospital care for patients with chronic conditions who are frequent users of acute care services. Assists patients and their family and/or support systems with coping skills as it relates to patient's chronic illnesses, and helps patients in recovery adjust to the emotional and practical requirements of life. Serves as a mentor to student health coaches. Must be willing to work out of Care4U Fontana and Upland locations as well as Ontario Urgent Care.
MINIMUM QUALIFICATIONS
Education: Masters Degree (MSW) or higher (LCSW), from an accredited School of Social Work.
Experience: Minimum of two to five years of hospital, medical, and/or field experience required that demonstrates advanced skills and knowledge.
Knowledge and Skills: Knowledge of medical and behavioral disorders symptoms and treatment. Ability to evaluate/assess patients with medical and behavioral disorders and develop safe self-care plans and recommendations. Ability to comply with mandatory reporting requirements. Knowledge of the relationship between medical illness and behavioral health problems related to short and long term interventions and solutions. Ability to conduct a variety of social work assessments and interventions. Knowledge of community resources.
License/Certifications: Current American Heart CPR card; Licensed to practice in the state of California (LCSW) preferred
Equipment: Computer literate; Proficient with Microsoft Office Software.
Physical Requirements: Must be able to perform the essential physical requirements of the job.
PAY RANGE
$29.96 - $44.94
The posted pay range reflects the lowest to highest pay that was available for this position at the time of posting and may be subject to change. Salary offers are determined by candidate’s relevant experience and skills. For per diem positions, a standard rate is used based on market data and not the candidate’s individual experience.
